Depending on distance from ground zero one would either be instantly vaporized or die from the shock wave. Those on the fringe of the blast area would suffer from penetrating beta and gama radiation producing burns and damaging bones and internal organs. Those more distant may suffer radiation sickness from ingesting or inhaling alpha particles. Some would survive their exposure to beta and gama radiation and most would survive the effects of alpha radiation. A significant percentage of survivors would suffer the latent effects of exposure in the form of cancer, leukemia, etc.I'm writing this from memory but I believe it's fairly accurrate
